(a + c) / (b + d) — the wrong way that works
The mediant of a/b and c/d is what happens if you add numerators and denominators separately — exactly the schoolchild mistake. The result is not a/b + c/d, but it is always a fraction strictly between them, and always already in lowest terms when the parents are. From this single bad addition, an infinite ordered structure falls out.
